RELEASE OF LIABILITY AND INFORMED CONSENT FORM

I, the undersigned, represent that my participation in the Wellness program is voluntary and hereby give my full irrevocable consent to the use of my Sensitive personal data or information as specified in this Release.

My Sensitive personal data or information will not be shared with my employer however my employer may be advised of the fact of my participation in the Wellness program.

UNIVERSAL: (Grants you access to all the Wellness service program components. Your sensitive personal data or information will not be shared with your employer, however, it would only be used for the purpose of data aggregation in a de-identified form.)

I understand my Sensitive personal data or information may be accessed from the GE India MediBuddy Portal of Medi Assist Healthcare Services Private Limited and used by the Wellness Coach (a wellness expert for the purpose of review analysis of relevant health {including health check and health risk assessment reports } and lifestyle information of an employee.) to provide specific recommendations related to health promotion and/or ailment management services including data aggregation for program improvement purposes and goal setting based on the employee’s identified risk and/or ailment and that such information shall not be used for any other purpose other than for the use as specified in this release.

RESTRICTED: (Grants you access to all Wellness service program components. Your sensitive personal data or information will not be shared with your employer and shall not be applied for the purpose of data aggregation.)

I do not consent to the access and/or usage of my Sensitive personal data or information from the GE India MediBuddy Portal of Medi Assist Healthcare Services Private Limited for the purpose of such Wellness program components involving data aggregation (in whatsoever manner and howsoever called) whereunder my personal sensitive information is aggregated, de-identified and applied to such Wellness program components.

I limit my consent exclusively to the access and/or usage of my Sensitive personal data or information for such Wellness program components that do not aggregate my personal sensitive information for whatsoever purpose and I understand that such information shall not be used for any other purpose other than for the use as specified in this release.

The importance of safeguarding Sensitive personal data or information is recognised and all persons viz. natural or juridical involved in this Wellness program are obligated and shall take reasonable steps to protect such Sensitive personal data in accordance with The Information Technology Act, 2008 as prevailing in the Republic of India and the Rules (as applicable) made thereunder or information from unauthorised access or use.

I hereby release GE India and all its legal entities, my employer and any other juridical person(s) including Medi Assist Healthcare Services Private Limited associated with this Wellness program, their affiliates, directors, officers, employees, agents, representatives, subcontractors, successors and assigns, from any liability arising from or in any way connected with my participation in any of these program components [voluntary health check-up, health risk assessment and wellness Coaching session(s)].

I understand that:

The data derived from the Wellness program are considered to be preliminary; they are to provide a current health status assessment only. They are not intended to be construed as medical diagnosis or confirmation of any ailment nor implied to be a substitute for professional medical advice.
The responsibility for initiating a follow-up examination to confirm the results of the voluntary health check-up and obtain professional medical assistance is mine alone, and not that of any other persons (juridical or otherwise) associated with this Wellness program.
The health and/or lifestyle recommendations advised by the Wellness Coach are based upon the identified risk(s) and/or ailment(s) and I hereby provide consent for working towards achieving the goal(s) set to mitigate or maintain the risk(s) and/or ailment(s).
